Coronary heart disease is damage to the heart caused either by a reduction or by the cessation of the blood supply to the heart, which is caused by pathological changes in the coronary artery system of the heart.

What is a heart attack?

A heart attack or acute myocardial infarction is damage or death of heart muscle cells (that is, the myocardium) that occurs as a result of circulatory disorders. This is one of the most common causes of death in almost all developed and developing countries.

For example, in Serbia, about 10,000 people experience a heart attack every year, while acute myocardial infarction is characterized by a huge mortality rate. Statistics show that about 30% of patients die, while almost half of that percentage do not reach the hospital.

Who has a myocardial infarction

Statistics show that this disease mainly affects men, more precisely than women – although after entering the climacteric phase, the frequency of heart attacks is almost equal in both sexes. Certainly, to a large extent, the black statistics show that more and more patients are younger than 40 years old.

A heart attack usually occurs in the morning hours, while, according to statistics, almost half of them occur after some extremely strong physical activity or some emotional stress.

Some of the main symptoms are: chest pain, tightness in the area behind the sternum that slowly intensifies and spreads towards the left arm, lower jaw or stomach.

The so-called silent heart attack occurs in elderly people or patients with diabetes.

Other symptoms include suffocation, nausea, vomiting, loss of consciousness, weakness and sweating.

What to do in case of a heart attack?

If you feel the signs we have listed, it is very important to take a comfortable position and call a doctor immediately . Under no circumstances should you try to walk to the specialist’s office, but don’t try to drive either.

The fate of the patient depends on the size of the region of the heart muscle affected by tissue death, as well as the time it takes to open the blocked artery. That is why it is very important that a person with symptoms be taken to a doctor or to a cardiology center as soon as possible in order to receive appropriate medical procedures.

Considering that half of the people who experience a myocardial infarction die, while the vast majority die even before reaching a doctor – it is very important that the warning signs are taken very seriously and that you react in time to seek adequate medical help.

Myocardial infarction – causes of occurrence

At the very beginning, in a sentence or two, let’s start with the theory if you have forgotten it. So we all have two heart (coronary) arteries – left and right. The changes that occur precisely in these blood vessels occur due to the so-called atherosclerosis, which affects the arterial system.

Atherosclerosis in itself is a process that occurs mainly with aging, but it is influenced by many other factors that we mostly hear are cardiovascular risk factors. It is precisely in their presence that the probability of developing a myocardial infarction is greatly increased.

Some of the cardiovascular risk factors are: diabetes , smoking, high blood pressure, genetic predisposition, age, elevated levels of various types of fats in the blood, obesity, physical inactivity, etc. Statistics also show that men are generally more at risk when it comes to this disease.

Indeed, some of these factors that we have mentioned can be influenced, but some cannot. On the other hand, the presence of several factors that represent a cardiovascular risk increases the chance of developing coronary disease in a form that includes myocardial infarction.

The most important thing is to monitor the symptoms regularly, that is, to react in a timely manner. Pain, which is the most common symptom, usually occurs in the morning or after some physical activity, a large meal, or when it is cold or after some stressful situation. In patients, the mentioned pain spreads to the left shoulder, left arm or lower jaw; and the location itself is behind the sternum, it is very intense – while patients mostly describe it as tightness, burning or tearing in the chest.

The pain itself in the infarct state lasts over half an hour and is generally much stronger and longer than all previous attacks the patient has had; of course, if the patient already had them. The pain itself can be accompanied by profuse sweating, sudden weakness, as well as malaise and suffocation (more precisely, a feeling of lack of air). What is also expressed at this moment is a strong fear of death.

On the other hand, in patients with diabetes, pain may be absent, and then only other symptoms may indicate that the patient has a myocardial infarction.

Therapy for myocardial infarction

The first thing to do when a myocardial infarction occurs is to stop the activity that led to that condition as soon as possible. Some of the first steps are putting nitroglycerin under the tongue, but also contacting medical institutions as soon as possible. It is very useful (for example) in such situations to chew one Andola tablet (of 300 mg). When the emergency services arrive, they act only on their advice.

In order to determine the presence of a heart attack, the doctor will perform an ECG examination as well as a specialist laboratory analysis . People who have had an acute myocardial infarction can be helped the most if less than 2 hours have passed since the pain began. It is precisely during this period that the administration of specific drugs has the greatest effect, and the patient can receive help in the Catheterization Room, as well as by opening a blocked artery and placing a stent.

The medical profession agrees that after a myocardial infarction, it is necessary to take certain types of drugs for life, which the doctor will prescribe after all the tests.

It is very important that in the first few days after the heart attack the patient rests, and that the rehabilitation process is started gradually. It is very important for all patients to have a coronary angiography in order to make the right decision about the further treatment, which can be in the form of continuation of medical therapy, surgery on the blood vessels of the heart (the so-called baj – pas) or the installation of stents in the coronary arteries.

The zone of the
